Referring

The easiest way to refer is via our website.  Use the Refer a patient button on the top right hand side of the website.

If you register yourself, it will make it easier for future referrals. On the form fill in the requested information.  You can also put any other details you wish but this is not necessary. We do ask you ensure all patient details are correct at the time of referring, as we do get many through that are not. Mobile numbers are very helpful for us to have, if you can provide them.

Once the online form is sent, you will get an immediate response saying that it has been received, on the screen. If you wish for a copy of the referral you have just made, before sending it, tick the box at the end of the page and you will have it emailed automatically to you.

If you prefer to send a letter or a paper referral, thenyou can download the pdf version of the form and send it to: Witney Brace place, 15 Langdale Court, Witney, OX28 6FG. You can also call the practice and request for some referral pads to be posted out to you, if you wish.

Your patient will be contacted as soon as the referral is received to arrange an appointment.

ADVICE:  Parents and patients can sometimes get annoyed when they have apparently been told that they NEED treatment because it will lead to problems later in life and/or the treatment will be available on the NHS, and then we tell them the exact opposite.  The simplest thing to do is avoid any mention of whether braces will be on the NHS and just tell them that, if they want, you will send them for an opinion so as to discuss braces.

There is currently no waiting list to be seen or for treatment to be started.
